I thought I removed my ex husband's name from my info, but it keeps referencing him. How can I be sure this is MY return only?

You need to create a new account and start fresh. There's no good way to turn a former joint account into a single account.

Yes, if he was listed as the primary taxpayer last year you MUST start a new account, there is no way to change your position from spouse to primary taxpayer cleanly.
