Yes, the military provides a Form 1095-C (for Active Duty Military and Federal Civilian Employees).

These forms will be available through your online DFAS myPay account access system on 13 January 2016, if you have elected electronic tax statement distribution.  If you receive tax statements through the mail, they will be mailed between 10 and 31 January 2016.

While you need to keep a copy of your Form 1095-C for your records, either electronically or on paper, you don’t actually have to submit the form to the Internal Revenue Service (IRS) with your income tax return.
