I signed up for Medicare in 2016 ending my recognized health care sharing ministry type D form 8965 exemption. How do I file this?

Do i need to apply for a 2016 exemption for January thru July  while I was on my exempt plan? I got an exemption in 2015, does this have to be renewed every year?

You don't need to "apply" for that exemption at Healthcare.gov, it is just entered on your tax return.

You will tell TurboTax that you had health insurance for PART of the year, then check the months that you had Medicare.  Then it will ask you if you had any situations that qualify for waive the penalty.  One of the check-boxes says a Healthcare Sharing Ministry.   Check that box and check the boxes for January through July.
