You should contact your employer and request a W-2. You are an employee and they are required to provide you with a W-2. Taxpayers who are unable to get a copy from their employer by the end of February may call the IRS at [phone number removed] for a substitute W-2. The IRS will send a letter to the employer on the taxpayers’ behalf. This link IRS Can Help Taxpayers Get Form W-2 has information that you may find useful.
