I received a separate W2 from my employer for having had Covid and it was paid through the government at the time. How do I input this on my taxes?

Please clarify, 

Box 13 on a W2 has three check-boxes in it. Is this where the information is listed?

Why did they give you a separate W2?  Is the EIN # at the top the same?  Did you mean box 12a instead of 13?  Is anything else listed on the W2?  It might be just a continuation of the first W2 if they ran out of room.  If it is you can enter the extra info on the first W2.  You can add more rows for boxes 12-20.  For box 12 just enter it in the next row or box.  It doesn't have to match the box 12a-d etc.
