I received a bonus check in Dec 2017. It’s not reported on W2 and I didn’t get a 1099. All taxes were withheld from bonus except FEDERAL. How do I report this on return?

What kind of taxes were withheld? Social Security, state taxes?

If your social security taxes have been paid, you can add the bonus income to line 21 of your 1040. 

In TurboTax under Income and expenses, select Less Common Income, Miscellaneous Income, then Other Reportable Income. Type in the description and amount. Item will then flow to line 21 on your 1040 and taxed with your other income items.

If you had state taxes withheld, then select Deductions & Credits, Estimates and Other Taxes Paid, Other Income Taxes. Scroll to bottom of page and enter any withheld state taxes in the section 'withholding not already entered on a W-2 or 1099'.
