I received a bonus check from my employer that was not reported on my W2 or a 1099. Taxes were withheld on bonus pay stub EXCEPT for federal. How do I claim this income on my return.

Ideally, you would contact your employer and have them issue a corrected W-2.

If you can't do that, then after you enter the W-2, check the box for "my W-2 is incorrect or I did not receive one" on the page of special circumstances that comes after the main W-2 page.  Enter the corrected numbers as if the bonus and taxes were included in your original W-2.
