I'm on SSA for last 3yrs and have a part time job 4.5hrs a day. They have me as a contractor and do not take taxes from my check. What form do I use? Am I self employed?

If you received form 1099-MISC you would be considered self-employed (independent contractor). You would have to file a Schedule C with your tax return.
