Use the one that has a W by it.

W = Employer contributions to an employee's Health Savings Account

Your W-2 has 4 separate box 12 entries (boxes 12a through 12d). Enter each one in one of the W-2 box-12 boxes onto TurboTax's W-2 form. Enter these amounts nowhere else in TurboTax.
