I have to mail my return because my husband had disability income with W2 from Hartford with Box 12 a J income - do I have to provide a copy of the W2 with my return?

Yes, when you filed by mail, all W-2's need to be attached along with any other documents that had income tax withheld (1099R's, SSA-1099).

The first page printed, has instructions, on what needs to be attached with the mailing address.
