I have to mail in my federal and state tax. What forms do I have to sign and what else do I have to include in the mail?

I have to mail in my federal and state tax. I print out the returns, sign/date in form 1040 for federal, form 540&1040 for state. Do I have to sign/date any other forms in the printout?

Turbo tax also prompts me to include a copy of W2. I have 2 questions: (1) Do I have to include any other forms? I have 1099B,1099DIV,1099INT,1042S,1098T as well. (2)Do I need to include those copies for both Federal and State?

The first page in your printout package will be the File by Mail Instructions, which will provide the mailing address and what forms, you need to include.

the federal tax return print out did not any instruction paper.

I'm sorry for the confusion. They primarily want forms attached to the return that show income tax withheld. Below is the official instructions.  If your 1099's don't show any income tax withheld, you do not need to attach them.

Assemble Your Return

Assemble any schedules and forms behind Form 1040 in order of the “Attachment Sequence No.” shown in the upper right corner of the schedule or form. If you have supporting statements, arrange them in the same order as the schedules or forms they support and attach them last. Do not attach correspondence or other items unless required to do so. Attach Forms W-2 and 2439 to the front of Form 1040. If you received a Form W-2c (a corrected Form W-2), attach your original Forms W-2 and any Forms W-2c. Attach Forms W-2G and 1099-R to the front of Form 1040 if tax was withheld.
