I have three 1095-B health forms to input I don't see my "plan" as a choice, and the "what if i don't see my plan type" is not loading....

Hi - you don't need to enter the type of insurance you have. 

• 1st page of health insurance: Indicate that you had coverage all year (or the period of time that all 3 of your 1095-B cover)
• 2nd page: select no to the question "Was anyone covered by any of these plans in 2016?"
• That's it!
