I have been asked for a "completed form W9" to a company paying me as an independent contractor - I thought the W9 is the application for an EIN - what should I provide?

I have both my original application as well as form SS-4 that the IRS provided to me (as well as Change of Address forms).  Thanks for any help!

A W-9 is NOT an EIN application ... that is an SS-4.   They need your company/personal info to they can properly issue you a 1099 next January.  The W-9 is pretty straight forward fill in the blank form ... read the instructions.
