I HAVE A 1095-A FORM AND RECEIVED A PREMIUM TAX CREDIT. DO I ENTER FOR ON MEDICAL INSURANCE PREMIUMS WHAT I PAID OUT OF MY POCKET OR ENTER AN AMOUNT FROM THE 1095-A FORM?

I paid $375.13  X 12 months in premium payments last year totaling $4,501.56, this is less the premium tax credit.  Do I enter this amount as my Medical insurance premiums?  Or do I use the total on the 1095-A form?

You can enter medical insurance premiums that you actually paid. This would be less the premium tax credit.
