I got married in September, 2015 and my wife had a different last name before that. If she got a state refund last year, how can I enter that under her name?

If your wife has notified the Social Security Administration of her name change, it will not make any difference if the state refund paid to name was her previous name.  If she has not notified the Social Security Administration of her name change, then she should do so right away.  Even if she has not, the name difference will probably not make any difference as long as the Social Security number is correct.

Do I enter my wife's state tax amount as an additional one for me, how do I show her name on it, since she filed on her own last year?

Did she itemize her deductions, or use the Standard Deduction?  If she took the Standard Deduction, you do not need to report her state tax refund.  If she did itemize, you need to report it.  There will be a box to check to indicate that it is the "Spouses" Form 1099-G.
