Sounds like they withheld state tax that year but did not report it correctly on your W-2.
You can look at that year's state tax return, you might get a refund if you file an amended return.
It does not affect this year's returns, neither state nor federal.

I transferred to a new state with that company two years ago and I left that company in July. Could it be related to me rolling my 401K from that company?
It is unlikely. Box 12 Code DD is where 401k contributions are reported.
All income is reported in the year received because the rules apply to that year specifically.
For 401k distributions when it is rolled over, you would get a Form 1099-R with a Code G in Box 7. This is not a taxable transaction, but should be reported in your return.
To enter, from the left menu>Federal>Wages & Income>,
scroll down to find Retirement Plans and Social Security>
Click Show More>IRA, 401(k), Pension Plan Withdrawals (1099-R)IRA, 401(k), Pension Plan Withdrawals (1099-R)> Click Start or Revisit.
