TurboTax failing to include the explanation for Spouse's Form 5329 requesting a waiver is a bug in TurboTax that has been previously reported.
Your present options are to include Spouse's explanation on the same explanation as Taxpayer's explanation, or paper-file by mail and add a separately prepared explanation.

TurboTax will create a form 5329 for each of you if you both had an early distribution from your retirement accounts [5329-T and 5329-S].
Be sure to mark the 1099-R entries correctly to identify that the distribution belonged to you OR your spouse.
