I entered that I needed another 1099-r, I did not need this and it telling me to enter zip code,state and city,i have nothing to enter,what do I do?

Go back to the Wages & Income, 1099-R section. You can delete the 1099-R that you created when you said you needed another 1099-R. Once you delete that 1099-R, TurboTax won't ask you to enter the zip code, state, and city.
