You would have a 1095A if you had marketplace health insurance---aka "Obamacare" aka Affordable Care Act aka healthcare.gov. If you have Medicaid, you do not have a 1095A so when it asks about 1095A say NO and just move on.

If you have Medicaid then you will not receive a Form 1095-A. When asked about that form, just answer NO>
