I am so frustrated that I could swear. How do I file a w-9?? When I asked my employer, they told me to try first...which I had. So I don't want to go back.

Your employer should likely give you a W-2. The W-9 is an information form given from 1 party to another to provide the giving party's tax filing information to include the proper name, entity type, address and TIN (FEIN or SSN); this is generally used for vendors and independent contractors.
