I would like to deduct her insurance premiums because I am self-employed on my 1040 form rather than as an itemized medical expense on Schedule A. How do I know if I can do that? I made more money being self-employed than the premiums cost.

You don't qualify. To qualify for the self-employment health care premium deduction, the health insurance would have to cover you (although it may also cover spouse and family), and you cannot qualify to have insurance through your employer. Since this is not the case, you will not be able to claim the health care premium deduction on your return.
