Your disability income should be reported to you on a 1099-R form or on an SSA 1099 form. If it is reported on a 1099-R form, you enter it as follows in TurboTax:

  1. Income and deductions
  2. Retirement Plans and Social Security
  3. IRA, 401(k), Pension Plans(1099-R)

Just work through that section and enter the information off of your form 1099-R.

If the income is reported on a form SSA-1099, you follow the same instructions but choose the Social Security(SSA-1099, RRB-1099) option

You don't need to do anything special to report the 401-K contribution, just enter the information reported on your form 1099.
