What is the reject code or message? There can be many reasons, and our crystal ball is permanently broken
Thanks for responding, Mike. There was no error code - recovery instructions along lines of, 'there may be something wrong with your HSA entries - go to the deductions/HSA section and check entries'. (paraphrased). Deleting all and re-entering, and trying any possible minor variations of entries did not resolve the issue. Was able to reach phone support - after double checking for entry errors (none found), we checked the 8889 form. There 2 forms present, but we have only one HSA.... The superfluous 8889 has been deleted and I'm in process of resubmitting. When following the workflow prior, it's possible that I accidentally selected the HSA checkbox for my wife - suspect that may have generated the excess 8889 form. When I later removed that entry, TT seems to have left that 8889 in place. It doesn't generate a TT error, but IRS must pick it up on the electronic exchange.
The fix did not work - same error message (no error code). Next phone associate I spoke to could not find any issue. Suggested I try a different browser or contact IRS during the week to see if they have the return. Indicated he is escalating the issue for research. Also suggested I might file by mail - TurboTax checker finds nothing wrong with the return.

I'm also having the same problem. TurboTax says I have a customer service message, but there is no message and no error code.
I also am getting the same error. I called and talked to someone this morning that was unhelpful. His comment was that he has never seen that error before. I wasted money asking them for advice.
Thanks to others taling time to confirn this issue. I tried a different default browser, clearing cache and resetting router (suugested by Intuit).. no improvement. My understanding that the return passes electronically to Intuit's secure server, then Intuit nteracts directly with the IRS Portal. Since Turbotax finds no error with the return, one suspects the rejection occurs between Intuit and the IRS. There's no timeline on resolution, so I'm filing by mail and requesting refund of the e-file fee.
OOOH - I was on the phone with Intuit for over an hour and Turbo Tax definitely has a glitch. If you find your Form 8889-T and scroll all the way to the bottom, it appears that, in Part III, Line 18 Smart Worksheet, the "None" box for December is checked off, indicating Medicare was received. This happened on two different tax returns that I did! That box should not be auto-checked. Also, be sure that, if you don't have a spouse, form 8889-S is not in your form list. I needed to delete that for one of the returns. Hope this helps someone.
Well, the return was still rejected after all of that!
Thanks for that suggestion about line 18. Our return originally included the incorrectly present spouse 8889 form also (which I deleted a few failed submittals ago). Just checked our 8889 for the line 18 worksheet issue (also the line 3 similar worksheet for any mistaken entries). No errors here (so it doesn't explain our rejection). Our return doesn't require 8889 Part III - lines 17 and 18 are not populated (line 16 is the last entry).
If you are getting the HSA_2025 rejection code when trying to electronically file your return, try using the following steps to clear the internal condition that is causing the rejection code. These steps have worked for some users to be able to refile with acceptance of their returns.
Also, please note that you should not check the box indicating that you are ‘covered by Medicare’ unless you are receiving Medicare benefits rather than having HDHP insurance coverage. This question is also asking about 2025, not 2026. So if you are covered by Medicare in 2026, this check box does not apply to you.

HI, appreciate the suggested fix. I followed it closely and resubmitted. It didn't solve the issue. TurboTax checking routines report no error, but the submission still comes back rejected with the same error messages (and no specific error code). I've checked to confirm that the TT workflow entries pass through correctly to Form 8889 - there is no mismatch.
Just confirming - my situation has no Medicare and no spousal HSA. We were covered by my employer's HDHP and only my one HSA the entire year (and the prior year). I did have to remove (delete) an 8889 related to a non-existent spousal HSA about 6 rejections ago (as suggested by TT help line). I'm wondering if there may be some vestige of that incorrect HSA entry somewhere (?) There's no entry in the workflow that would explain it, but if there may be some other internal table not visible to users (that's isn't resetting correctly)...
After about three hours on the phone with Intuit today and getting nowhere in resolving the rejection issue (submittal has been rejected 6 times now), it occurred to me that my HR department has an incorrect address on my W2 AND on my HSA forms and perhaps this discrepancy is what's causing the rejection at the IRS -- since the address on my 1040 does not match the address on my W2 (HR literally got my house number wrong but, strangely, I still received them in the mail!). I realize this is pretty far out there in left field, but does anyone else have any sort of similar mismatch between their W2 and their 1040? I'm suspecting that this might be the cause of the rejection, so plan to request that HR provide amended W2 and HSA forms so that they go to the IRS correctly. Just another guess...
