How should I file a bankrupt stock? I owed a stock that was on Nasdaq board in 2015 and then it was delisted in Nasdaq 2016 and eventually went bankruptcy in 2017.
I am not really sure what cost amount I should put in the filing as the stock changed to a different symbol once it delisted to OTC.

You will need the cost basis when you paid for the stock. If you don't have that handy, you will need to estimate the cost in a reasonable manner. Google, morningstar, etc have historical stock prices that you can research.
Enter in the Investment section with a sales price of zero.
