What does a bi-vocational pastor's W-2 look like if he has no clergy income but gets a housing allowance?

A "housing allowance only" W-2 has an entry for the housing allowance in Box 14, and no other boxes are generally completed.  This W-2 must be marked as being for clergy in the TurboTax followup asking about special situations for the W-2, or it will cause errors in your return.
