If you took more than your Required Minimum Distribution (RMD), you don't report the RMD separately from the total distribution.
Report the full amount shown on Form 1099-R.
The IRS doesn't require you to break out the RMD portion versus the excess amount. Any amount withdrawn above the RMD is just an additional distribution and is taxed the same way.
