if your employer paid you the bonus, it is not being reported properly if it's not on your w-2.

the right thing to do and what the IRS wants you to do is to request a corrected w-2 but that means possible issues with your employer.

your other option is to report on schedule C - then 100% of fica and medicare taxes come from you.

or you can use form 4852
