If you have state withholding on your W-2 or other income form, but the issuer didn't enter the state ID number, contact your employer or other issuer for the number. Each state may have its own criteria for the format of the number.
As a workaround, if you can't get the correct number, you can enter a placeholder number such as the Federal ID number, or a series of 9s. TurboTax may tell you in Review if the number doesn't have the correct format for the state, or you may find the format information at your state Department of Revenue website.
Or is it for a state without an income tax?

...if so, you leave boxes 14, 15, 16 entirely blank on a 1099-R
