Why are you asking what tax form you filed? All personal tax returns are filed on Form 1040. There is no other form for personal tax returns.

(If you were 65 or older you might have filed Form 1040-SR, but Form 1040-SR is exactly the same as Form 1040 except that it has larger print. It makes no difference whether you filed Form 1040 or Form 1040-SR.)

A transcript is not a copy of your tax return. A transcript is a document you get from the IRS that lists the information that was on the tax return that you filed, but not in the format of the actual tax return forms.
