If your name on your social security card has two middle names, in order to e-file you will need to include your second middle name as part of the last name as you stated above. This is because the IRS automatically assumes that the third name on your Social Security Card is your Last Name.  

For example, if the name on your card is Sara Meredith Judith Jones, you would enter the name as follows:

First: Sara

Middle: Meredith

Last: Judith Jones

This is helpful to know, but I did want to clarify that I have 2 first names, a middle name then 1 last name. I just wanted to make sure that if I do put my middle name as part of my last name, it isn't wrong because I realize that my official last year's tax return when I download it shows my last name including my middle name and then in the box where it says first name and middle initial, it just says my first name. I just want to be cautious that I am filling it out correctly, but if I can do the same thing I did last year then that's okay.
