You can do it using your last pay stub. You will just need to enter the .5 of your OT. So if your employer breaks your paystub down by Regular and Premium, you would use the premium amount. If they just give you a lump sum of what you earn for OT without breaking it down, then you would divide that by 3 to get the OT Premium amount. If you are paid double time you would divide it by 4. Only the .5 amount is deductible.

I have the same question and you didn't answer it at all. This is about tips that were reported to our employer. The employer put all cash and credit card tips in Box 1 with wages and did not separate it. How do we claim a deduction for reported tips that are lumped in with wages.
When your employer adds tips to box 1 of your W-2 and didn’t list them in box 7 or 14, you will have to determine your tips. Make sure that the amount matches the amount that you reported to your employer.
