Health Insurance Medicaid

I have a totally disable child (4 years old) I have health insurance through work and he is on that but he is also on Medicaid, I get a reimbursement from Medicaid  for the cost of him on my work policy, (1) I get no end of year paperwork how or do I need to show this amount anywhere, and in the healthcare question it asks if anyone is on State Medicaid if I check yes it does not give me the option to just show it is for him, do I not check it off?

Show him on your work health insurance, not Medicaid. 

Although you are reimbursed, your paperwork will have him covered by your employer. Your tax return should match the documentation.
