Is this your issue?

The Utah Payer/Employer state withholding account number has 14 characters--11 numeric characters followed by "wth" (e.g., wages) or "wmp" ( if mineral withholding).  Do not use any hyphens, and do not use any capital letters.  If it's having you correct the number, use lower case for the "wth" as in this example:

12345678901wth

It needs to be in that format on all of your W-2 forms and also any 1099 forms that has a Utah state withholding number.
