Forms to submit when filing uneven quarterly est. tax payments for self-employed?

Hi, I have a question about making uneven quarterly estimated tax payments for self-employed.

 

I am self-employed and have variable income. Rather than paying estimated taxes in equal installments, it will be easier for me to make four uneven payments (using the annualized method) by the quarterly tax deadlines.

 

From what I understand, the 1040Es form is only used for even installments.
 
For uneven payments,  it looks like I would need to submit the 2210 and 2210 AI forms with my end of year return to explain the uneven payments. But do I need to file a form *with* the payments each quarter? 
 
Or do I simply make the uneven payments, calculated by the annualized installment method, and file the 2210 forms with my year's end tax return?
 
P.S. Are there forms I would need to submit with uneven quarterly payments for state income tax (MI)?

You only need to submit the annualized statement explaining the uneven payments with your return at the end of the year.  

 

And the tax form for Michigan (also annual) is form MI-2210.  Easy to remember.

you can use the 1040-ES for uneven payments. just cross out the numner that is there and enter the number you want. It might be easlier to set your self up to do electronic payments bu setting up an ID.ME account.

 

if you mail use a tracking service and mail early. think of the problems you would have it got lost or did not arrive timely!

with e-payment you can do it ahead of the due date and specify the date for payment and the amount. You can also print or save a receipt so you know it's been taken care of.
