Hello - I rolled over the entire balance in my 401K to a rollover traditional IRA account. The distribution of 401K included "cash" and a couple of stocks. The 1099R for the 401K distribution, for example, show $50,000 was moved out of the 401K into a rollover IRA, which is not a taxable event. However, the "stocks" that were distributed out had a price increase change of $100 when the rollover was settled into the rollover IRA account, so the form 5498 for the rollover "IRA contribution" show a different amount, say $50,100, than the form 1099R that shows the "401K distribution." of $50,000. I understand that the 401K and IRA custodians send both the 1099R (401K distribution) and 5498 (IRA contribution) to IRS. How do I report the $50,000 401K distribution and the $50,100 rollover IRA contribution on the tax return? I understand that the 401K distribution of $50,000 would be on line 5a (pension and annuities) on form 1040, but where would the rollover IRA contribution of $50,100 go? And how do I account for the $100 difference between the 401K distribution and rollover IRA contribution that was due to the stock market price changes? Also how would I enter the data into TurboTax Premier Windows version?

You report the amount that is on the Form 1099-R. To enter this in the Desktop program:

The 5498 is only an informational form. Nothing from it is entered on your tax return.
So to confirm, there is no where in TurboTax, or on the tax return, that I need to enter form 5498, and I don't need to worry about (or reconcile) the amount on form 5498 is $100 more than the amount on the 1099R for the same rollover (401K rollover distribution per the 1099R and receipt of the rollover per form 5498)?
