For box 15 on my W-2 all that is shown is the state. The Employer's state ID number is blank. What do I do?

If box 17 is blank then leave all boxes 15-17 blank. What state? Some states don't have an income tax so leave 15-17 blank.

Block 15 only shows the state (GA) it does not show the Employer's State ID number. Block 16 and 17 are filled (I did have state taxes withheld) I can't complete my taxes without that number and I can't figure out how to find the number, I've searched all over the internet.
