Filled out HSA info and it created form 8889. Line 9A has amt employer contributed (amt on line 12B on W2). Amt consists of both my contrib and the employer contrib

That's correct. Code W is called employer contributions, but it refers to all of the pre-tax contributions that have come through your employer to the plan administrator. The Code W means that the contributions have already been deducted from income, so you will not deduct them a second time.
