Filing Taxes using a SOC-2298 amd W-2 Form

Hello, how do I go about filing my taxes using my SOC-2298 and W-2 form from IHSS (caregiver)?  Even though my wages can be withheld from having to file federal and state income, I still need to file for my health insurance. How do I go about that correctly and filing it correctly?

The SOC-2298 is a form you submit to California Dept. of Health and Human Services.  Keep a copy for your records.  Here's more info from CA Dept. of Social Services.
