I need help understanding how to handle a possible missed federal extension and a 1099-R / Qualified Plan Loan Offset issue.
I prepared my 2025 tax return using TurboTax around the original filing deadline. My TurboTax-generated New Jersey return shows “Federal extension filed,” and TurboTax also generated a New Jersey NJ-630-V extension/payment voucher. I paid the New Jersey amount by credit card on time and have the payment confirmation.
For the federal side, I also created an IRS short-term payment plan for the estimated balance due. However, when I called the IRS, they told me they do not have a record of receiving my federal Form 4868 extension.
I cannot find Form 4868 or a federal extension acceptance confirmation in my TurboTax documents. My TurboTax account shows the return as “Printed,” not “Accepted.”
My questions are:
- If the IRS has no record of Form 4868, should I assume my federal return was not extended and e-file the return immediately?
- I have a 2025 Form 1099-R issue involving a Qualified Plan Loan Offset of about $20,000. I have not rolled over or repaid that amount yet. If the federal extension was not valid, does that mean the QPLO rollover deadline was the original filing due date rather than October 15?
- Since TurboTax/NJ documents showed “Federal extension filed,” and I made timely tax-related payments and set up an IRS payment plan, would this support a reasonable cause request for any late filing or late payment penalties?
- Should I file now treating the QPLO amount as taxable, and then address any penalty notice later?
I am trying to avoid making the return later than it already is, but I also want to make sure I handle the QPLO/1099-R correctly before e-filing.
