Depends on the state and city. Generally yes. You attach a copy of the federal extension, when you file your state and city return.

So do I need to print off the extension and send? Sorry this is my first time doing this. I am located in Ohio

Ohio does not require a separate extension, nor do Ohio cities. Do not print off the extension and send it to the state and city. If you mail a paper return, when you file, then you attach a copy of the federal extension request or approval. If you e-file state, there is a box on Ohio form IT1040 that TurboTax (TT) will check that states you filed a federal extension. The RITA (and probably most other city) forms also have the check box, but you still need to attach a copy, since city returns cannot be e-filed from TT
