Does medical expenses include the co-pay amount after the insurance did their part? Or only when I did NOT use my insurance?

Medical expenses include the amount you pay out of pocket (not from an HSA), whether that is a co-pay, a service that was not covered, or an amount you had to pay to reach your deductible. 

You can't deduct the amounts paid by your insurance, or that were reimbursed by anyone else.
