Do you have an amount on your W-2 box 14?
Form IT-225 is used to report any New York additions and subtractions to federal adjusted gross income that do not have their own line on your return.
If any federal Form W-2 wage and tax statement shows that an amount was deducted or deferred from your salary under a flexible benefits program established by New York City or certain other New York City public employers on your or a decedent’s behalf, then you need to enter that amount.
I've never worked in New York City. Why would this specific text box require something other than a 0 or blank space?
Sometimes NY paid family leave appears in Box 14 W-2 , which is not Code A-101, but the program seems to categorize it as an IRC 125.

Also, if you are getting the error message shown in the image below, and it comes as "0", delete both of them. Before clicking "Continue", remove both "0's" as shown below
