This is a credit for EMPLOYERS (not employees).  Eligible employers are entitled to tax credits for wages paid for certain leave taken by employees related to the COVID–19 pandemic to recover from any injury, disability, illness, or condition related to the vaccinations.  You are asking from TT Self-Employed.  Did you have W-2 employees of your own in 2021?
