Does a form 1099-INT from the Dept. of Treasury, have to be entered as income? Even though, It does not have boxes like my other 1099-INT?

Yes, interest received from the Department of the Treasury does need to be reported as income on your tax return.

 

Go to the section where interest income is reported.  Enter 'Department of the Treasury' as the Payee, and put the interest amount in box 1, as if your form had reported it there.
