Do I need to report the SS # of the caregiver that stayed with me while my daughter was at work?

I am bedridden and need help to eat, use bathroom, activities of daily living etc.

Yes , in order to be able to claim the Child or Dependent Care Credit, you do need to have the Social Security Number or Federal ID number of the care provider.
