Yes, if you filed Form 8885, Health Coverage Tax Credit, you will need to mail it in attached to Form 8453 along with supporting documentation for the credit. 

You'll need to attach certain documents that prove you were eligible to claim the credit, that your 2017 health insurance plan was qualified, and that the premiums claimed for your 2017 coverage were paid.
