Do I have to fill out a W-9 for a check I received from my husband's long term care insurance if I am going to use it to pay his bill?

The nursing home costs were for Oct. 2017.  I just now received the check.  It doesn't pay the whole bill, so I will have to pay about 50% of the bill in addition to this  payment from his insurance. He passed away 12/01/17.

If the payer is requesting a W-9 from you, yes, you should provide one.  Filling this out does not affect the deductibility of his eligible medical expenses in any way.  After entering his expenses, you will be asked to enter any reimbursement amount paid by insurance or reimbursed to you by insurance.  You would enter this amount whether you fill out the form or not.
