1099-NEC only box 1 is filled with my income.
W2 box 1-12 empty, box 14 filled with CA-SDI amount, box 16 filled with same amount as box 1 on 1099-NEC, box 17 filled with a small amount.

The fitness studio should not be issuing both documents. You, yourself say you are an independent contractor which means there is no 'employee' income or withholding. A W-2 should not have been issued to you. The fitness studio should correct the W-2 by filing Form W-2C with the IRS.
As for your tax return, you should file the 1099-NEC as self employment income. The instructions at the link below will get you started. This will complete your tax return entry for both your federal and state returns. Do not enter the W-2, unless the fitness studio does have employee wages for you in addition to the Form 1099-NEC independent work.

You have to ask the employer how he classified you as. Are ytou considered an employee subject to his discretion or are you an independent contractor who has your own autonomy and flexibility. You may fall into both categories and be issued both a W2 for one set of duties and a 1099 for another set of duties. On the other hand you are saying the Box 16 on your W2 which is the state of Ca wages is the same amount as Box 1 of your 1099NEC. You must verify this with your employer and find out what he meant to give you and then we can go from there. Box 17 is the state income tax withheld.
