I am located in MA and took 26 week of leave (medical and family bonding). My employer uses Mutual of Omaha to distribute their leave payments. I received a w2 from my employer that has all my wages I physically worked, and then a received 2 other w2s from Mutual of Omaha that are my leave payments. Do I check the box for paid family leave on just the Mutual of Omaha w2s or on the one from my employer as well?

No, if your employer's W-2 does not include the family leave you should not check the family leave box. However, the W-2 you received from the third-party (Mutual of Omaha) provider should.
I am also assuming I will not receive a 1099G because my employer uses a private plan vs going through the state?
@mtheodorakis You are correct, you will not receive a form 1099-G if the benefits were not paid from a governmental unit.
