Do HSA bonus incentives count as an employee or employer contribution? ( In regards to contributions personally made)

If the bonus incentive is offered by your employer as a result of an action that you needed to take, it would be considered an employer contribution and should have been reported on your W-2 in box 12 with code W, along with your contributions that were made through your payroll.

Any contributions personally made by you would have been made outside of your company payroll, directly to the HSA.
